Using ICD-10-CM standards, codes, and abstracts medical records for reimbursement purposes. Integrates the 
hospital’s mission and “Guiding Principles” into daily practice.
ES S E N T I A L F U N C T I O N S
Essential functions are those tasks, duties, and responsibilities that comprise the means of accomplishing the job’s 
purpose and objectives. Essential functions are critical or fundamental to the performance of the job. They are the 
major functions for which the person in the job is held accountable. Reasonable accommodations may be made to 
enable individuals with disabilities to perform the essential functions.
1. Determines the appropriate code with the International Classification of Disease-10th RevisionClinical modification (ICD-10-CM); enters codes into billing software accordingly.
2. Reviews the entire medical record when applicable to assign the admitting, primary, and secondary
diagnosis to support medical necessity for all services performed.
3. Collaborates with medical records, medical transcription and hospital staff to improve the quality of
medical record documentation.
4. Completes physician query forms in accordance with department policy.
5. Assists with orientation and training of new staff when applicable.
6. Researches and or/ locates un-coded charts on unbilled lists.
7. Assists the Business Office with coding related questions, denials, edits and questions.
8. Participates in the government prospective payment system and ensures hospital adherence.
9. Generates required reports.
10. Performs other duties as assigned to support overall effectiveness of the department.
